Key Responsibilities\n

• Develop tailored treatment plans based on comprehensive clinical assessments, ensuring each plan aligns with the unique needs of the patient and their family.

• Actively participate in multi-disciplinary team meetings to contribute insights and recommendations that enhance clients' care pathways.

• Regularly monitor patient progress, keeping detailed case notes and following up proactively to assess treatment effectiveness and adjust plans as necessary.

• Engage with school staff, parents, and community resources to address the needs of student clients, ensuring well-rounded support systems are in place.

• Provide counseling sessions, utilizing strong verbal communication skills to offer guidance and support to patients and their families as they navigate end-of-life decisions.

Patient Population / Client Focus\n

In this role, you will focus on serving terminally ill patients and their families, providing essential support as they navigate complex emotional and psychological challenges during the hospice process. Qualifications\n

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State is required.

• A Master’s degree in Social Work from an accredited institution is mandatory.
