Key Responsibilities

The Physical Therapist will take on a diverse range of responsibilities that directly impact patient recovery and independence:

• Assess Functional Abilities: Conduct thorough diagnostic evaluations to identify each client's unique functional abilities and mobility challenges.
• Implement Treatment Plans: Directly administer targeted physical therapy treatments aimed at preventing disability, alleviating pain, and enhancing muscle strength and functionality in accordance with established care plans.
• Supervise Support Staff: Provide guidance and oversight to physical therapy assistants, ensuring compliance with treatment protocols and adherence to patient-specific care plans.
• Home Environment Evaluations: Assess home settings to recommend modifications that foster patient independence, and facilitate safe and efficient rehabilitation processes.
• Education and Guidance: Instruct clients and their families on proper usage of therapeutic devices, promoting empowerment and understanding in their rehabilitation journey.
• Interdisciplinary Collaboration: Actively participate in interdisciplinary team meetings to discuss patient progress, contribute to care plan reviews, and adjust treatment strategies as needed.
• Documentation: Maintain accurate clinical records of therapy sessions, progress notes, and treatment plans, ensuring all submissions meet regulatory requirements.

Patient Population / Client Focus

Qualifications

To successfully fill this role, candidates must meet the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Active Physical Therapy licensure in the state of California.
• A minimum of one year of experience in acute care or home care settings.
• Current CPR certification.
• A valid driver's license and automobile insurance with a reliable vehicle for travel to patient homes.
• Strong interpersonal and communication skills for effective collaboration with patients and interdisciplinary teams.
• Knowledge of federal and state healthcare regulations to ensure compliance in all practices.

Preferred Experience

While one year of experience is required, additional experience in specialized areas such as pediatrics, geriatrics, or neurological rehabilitation will be highly valued. Compensation and Benefits
