Key Responsibilities\n

• Conduct comprehensive assessments to determine the needs and progress of each patient, tailoring therapy sessions accordingly.

• Facilitate clear communication between supervisors, family members, and other stakeholders regarding clients’ treatment progress.
\n
• Establish and submit a tentative schedule to supervisors for upcoming appointments, home visits, and monitoring observations to stay on top of client progress.
\n
• Coordinate home visits, ensuring in-home observations are thorough and effective, and participate in ongoing chart reviews and data analysis.
\n
• Implement and monitor client program plans to ensure they are effective and align with expected therapeutic outcomes.
\n
• Engage actively in Interdisciplinary Team Meetings and contribute insights to improve collaborative care strategies.
\n
• Document clinical progress with high-quality monthly and quarterly notes, maintaining clear and accessible records.
\n
• Organize and lead training sessions to ensure staff are well-equipped with the skills needed to implement program plans and complete necessary documentation effectively.
\n
• Provide follow-up interventions, home visits, and ongoing assessments to ensure continuous support for clients.
\n
• Act as a liaison with community mental health organizations and the public to enhance outreach and development goals.

• Contribute to the creation of training materials designed to elevate the quality of our services and expand program efficacy.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must hold an active and unrestricted license as a Professional Clinical Counselor (LPCC) in the state of California.
