Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient care providers, ensuring seamless communication and coordination of care.

• Engage with insurance companies to manage pre-certification and utilization management processes, ensuring clients receive timely and appropriate services.

• Participate actively in discharge planning and aftercare strategies to support clients' transitions from hospice care.

• Provide therapeutic meal support as part of holistic care, using your expertise to model healthy relationships with food among clients.

• Deliver safe, empathetic, and affirming care tailored to the unique circumstances and intersecting identities of each client.

The focus of this role is primarily on serving older adults who are facing life-limiting conditions. Your work will directly impact their quality of life, helping them navigate emotional, psychological, and practical challenges during their care journey. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A current and valid LICSW license in the state of Massachusetts.

Compensation and Benefits\n
