Key Responsibilities\n

• Conduct patient intakes and perform diagnostic evaluations to identify mental health and psychosocial issues, recommending appropriate treatment plans.

• Collaborate with primary care providers to monitor patient progress and adjust treatment plans as necessary.

• Develop relapse prevention strategies to support stable functioning in patients, particularly those at risk of declining mental health.

• Engage in follow-up consultations to assess patient outcomes and make data-driven recommendations to enhance care.

• Document patient progress accurately, maintaining comprehensive and compliant clinical records as per agency and FQHC guidelines.

• Educate patients on self-management techniques and healthy lifestyle choices to empower them in their treatment journey.

• Participate in consultation discussions with clinic management to evaluate program effectiveness and areas for improvement.

• Adhere to ethical standards and professional codes of conduct throughout all patient interactions.
