Key Responsibilities\n

• Conduct thorough assessments to evaluate the mental health needs of each patient, ensuring that individualized treatment plans are developed and monitored.

• Engage in frequent communication with supervisors, direct care staff, families, and guardians to provide updates on patient progress and adjust plans accordingly.

• Develop and provide a tentative schedule for appointments, home visits, and necessary observations, ensuring progress monitoring aligns with treatment protocols.
\n
• Coordinate and execute home visits, which may include performing chart reviews, conducting staff interviews, and analyzing data to inform treatment efficacy.
\n
• Oversee the implementation of treatment plans ensuring that they are effective in meeting patients’ needs and making necessary adjustments as conditions change.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing insights and perspectives on patient care as required.
\n
• Document monthly and quarterly progress notes accurately to maintain a comprehensive record of treatment and outcomes.
\n
• Organize training sessions to certify that staff are proficient in the implementation of care plans and appropriate documentation practices.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to ensure continued adherence to treatment plans.
\n
• Maintain and manage records diligently, ensuring that information is accessible for authorized personnel and stakeholders.
\n
• Act as a liaison with the public and stakeholders in the mental health community, supporting development goals and initiatives to enhance service delivery.

• Assist in creating comprehensive training materials aimed at improving clinician competency and program delivery.

Patient Population / Client Focus\n

The LPC will work with a diverse client population, including both adults and youth, addressing a variety of mental health concerns, including but not limited to anxiety, depression, trauma, familial issues, and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of New Jersey is essential.
