Key Responsibilities\n

• Conduct thorough assessments to identify the mental health needs of patients, utilizing evidence-based practices to develop individualized treatment plans.

• Maintain clear and effective communication with supervisors, team members, family members, and guardians regarding patient progress and treatment adjustments.

• Prepare and submit a structured schedule of therapy sessions, home visits, and related activities to ensure continuity of care and monitoring of patient progress.

• Coordinate home visits to gather information through observations, chart reviews, and staff interviews, using a comprehensive approach to patient assessment.
\n
• Implement and monitor the effectiveness of treatment plans, engaging in regular evaluations to ensure high standards of patient care.

• Participate in interdisciplinary team meetings to discuss patient cases, share insights, and collaborate on treatment approaches.

• Document detailed progress notes on a monthly and quarterly basis, ensuring all records are accurate and readily accessible.
\n
• Develop and provide training sessions to ensure that all staff and team members are informed and competent in implementing treatment procedures and documentation standards.

• Engage in follow-ups with patients through interviews and home visits, reinforcing their support system and addressing ongoing needs.

• Act as a liaison between the mental health community and the wider public, promoting awareness and accessibility of mental health resources.

• Contribute to the enhancement of training materials and procedures aimed at improving the quality of our mental health programs.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in New Jersey.
