Key Responsibilities\n

• Conduct thorough initial assessments and ongoing evaluations to develop personalized treatment plans tailored to each patient’s needs.

• Maintain robust communication with supervisors, direct care staff, families, and other key stakeholders to monitor patients’ progress and adjust treatment plans as needed.

• Provide a timely schedule of planned appointments, home visits, and observations to facilitate effective care coordination.
\n
• Organize and execute home visits when appropriate to review patient environments and gather necessary insights through chart reviews and staff interviews.
\n
• Implement and oversee treatment plans, ensuring they are carried out effectively while providing ongoing support to patients.

• Actively participate in interdisciplinary team meetings to discuss patient care strategies, share insights, and collaborate with fellow professionals.

• Document detailed progress notes on a monthly and quarterly basis to track patient outcomes and report findings to relevant parties.

• Provide ongoing training and certification opportunities for staff to ensure competence in the implementation of treatment protocols.

• Conduct follow-ups through interviews and home visits to reinforce support for patients’ evolving needs.

• Serve as a liaison with the mental health community and the public to promote awareness and resources regarding mental health issues.

• Assist in developing procedures and training materials that enhance the quality of programs for individuals in need of mental health services.

Patient Population / Client Focus\n

Your role as an LPC will involve working with a variety of patient populations, including adults and youth grappling with an array of mental health challenges such as depression, anxiety, trauma, and substance abuse. The diversity of our client base allows you to employ your specialized skills in family therapy, couples counseling, and other therapeutic modalities, tailoring your approach to meet the unique needs of each individual.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• At least two years of direct experience in counseling, with a preference for backgrounds in clinical environments.

• Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly valued.

• In-depth understanding of legal and ethical standards in mental health practice.
