This position is primarily remote, allowing counselors to work from the comfort of their own space while offering outpatient services to clients in San Antonio, Texas. Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to identify clients’ mental health needs.

• Communicate effectively with supervisors, direct care team members, and family members to facilitate a cohesive treatment plan tailored to individual clients.

• Create and manage a tentative schedule of appointments, home visits, and follow-ups to monitor client progress and ensure effective treatment delivery.

• Coordinate home visits and conduct in-home observations, chart reviews, and interviews to gather essential data.

• Implement and monitor the effectiveness of treatment plans, making necessary adjustments based on client feedback and progress.

• Participate actively in Interdisciplinary Team Meetings to collaborate with various healthcare professionals and enhance the overall treatment approach.

• Document monthly and quarterly progress notes to track client development and inform future therapeutic strategies.

• Provide educational training for staff and team members on the implementation of program plans and documentation protocols.

• Facilitate ongoing support through follow-up interviews and observations aimed at promoting client welfare.

• Maintain meticulous records to ensure information is accessible to authorized parties while upholding confidentiality and legal standards.

Qualifications\n
