Key Responsibilities\n

• Conduct ongoing assessments to evaluate the mental health needs of patients and develop tailored treatment plans that facilitate their journey toward recovery.

• Maintain open communication with supervisors, family members, and other healthcare providers regarding clients' progress and treatment adjustments.
\n
• Organize home visits and ensure thorough documentation of individual progress through detailed chart reviews and observations.
\n
• Participate actively in interdisciplinary team meetings to ensure cohesive and comprehensive care for clients.
\n
• Complete and maintain accurate monthly and quarterly progress notes to document treatment outcomes and inform future interventions.

• Lead training sessions to ensure staff members are well-equipped to implement program plans and adhere to documentation requirements.

• Act as a vital liaison with public and mental health agencies, advocating for resources, and establishing pathways towards fulfilling developmental goals for individuals.

• Monitor and provide follow-up services, including interviews and further engagements, to support client needs continuously.

• Implement and coordinate continuing education to enhance best practices within the organization.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at our clinic, you will work with a diverse range of individuals from various backgrounds. Your clientele will include both adults and youth, catering to a variety of mental health issues such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A master's or doctoral degree in Counseling, Psychology, or a related mental health field is required to ensure comprehensive knowledge of psychological modalities.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Pennsylvania.

• A minimum of two years of clinical counseling experience with a proven track record of successful patient outcomes.
