Work Environment

This position is onsite and involves traveling to patients' homes within The Villages area, ensuring that you can provide therapy in a familiar and comfortable setting. Key Responsibilities

• Conduct comprehensive evaluations of patients' conditions and needs in their own environments, focusing on factors that contribute to recovery.
• Create and implement tailored treatment plans, utilizing a variety of therapeutic techniques such as exercises, hands-on therapy, and equipment to help alleviate pain and improve mobility.
• Monitor patient progress by meticulously documenting treatments and responses, adjusting plans as necessary to meet evolving needs.
• Collaborate with other healthcare professionals and caregivers to ensure a cohesive approach to rehabilitation and wellness.
• Educate patients and their families on effective self-care practices and the proper use of assistive devices to facilitate independence.
• Stay updated with recent clinical research and advancements in physical therapy by attending workshops and participating in professional organizations.

Patient Population / Client Focus

As a Physical Therapist at Hope Life Love, you will work with a diverse patient population, including children recovering from injuries, adults undergoing rehabilitation post-surgery, and seniors managing chronic conditions. Your role is crucial in empowering these individuals to regain functional independence and enhance their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy license in the state of Florida.
• Excellent verbal and written communication skills to effectively convey information to patients and families.
• Proficient computer skills for documentation and patient tracking.
• A strong commitment to patient care and ongoing professional development.

Preferred Experience

• A minimum of 2 years of clinical experience as a Physical Therapist in a home care setting is preferred, although new graduates with a passion for home health are encouraged to apply.

Compensation and Benefits
