Work Environment

This full-time position will require you to provide onsite home care, giving you the flexibility to deliver therapeutic services directly within the comforting environment of your patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients referred by healthcare providers, ensuring a comprehensive understanding of their physical and emotional needs.
• Develop and implement tailored treatment plans that are aligned with physician directives, utilizing modalities and therapeutic techniques that promote functional recovery.
• Provide ongoing assessment and modification of treatment interventions based on patient progress and feedback, ensuring personalized care.
• Deliver exceptional patient education; clarify treatment methods and motivate patients and their families throughout the recovery process.
• Monitor and manage patient progress and meet minimum utilization goals while contributing to the overall performance metrics of the practice.
• Collaborate effectively with a multidisciplinary team of healthcare professionals, sharing insights and strategies to optimize patient outcomes.

Patient Population / Client Focus

In this role, you will specialize in treating a diverse patient population with conditions spanning orthopedic, pediatric, neurologic, and sports medicine specialties. Your expertise will be vital in addressing individual rehabilitation goals and supporting patients in overcoming physical challenges. Establishing strong relationships with your patients is essential, as we aim to nurture trust and engagement, facilitating deeper connections that drive successful outcomes.

Qualifications

• Current Physical Therapy license in Virginia (DPT) from an accredited institution.
• Minimum of two years of experience working as a licensed Physical Therapist.
• Current CPR certification and knowledge of safety protocols.
• Strong verbal and written communication skills, with an ability to engage effectively with patients of all ages.
• Proficiency in using electronic health record (EHR) systems, along with basic word processing capabilities.
• A proven ability to build and maintain positive relationships with patients and their families.

Preferred Experience

While a license and relevant experience are essential, candidates with additional specialties in sports medicine, pediatrics, or neurological rehabilitation will find these competencies beneficial in enhancing the recovery pathways of our patients.

Compensation and Benefits
