Key Responsibilities\n

• Conduct thorough assessments and develop individualized treatment plans tailored to meet the diverse needs of your patients through virtual consultations.

• Maintain effective communication with supervisors, healthcare staff, and family members regarding patient progress, ensuring that everyone involved is informed and supportive of the treatment plan.

• Plan and coordinate regular appointments, home visits, and necessary observations to monitor patient advancement and adherence to the treatment program.

• Participate actively in Interdisciplinary Team Meetings and other relevant discussions to collaborate on treatment plans and address any emerging patient needs.

• Document patient interactions clearly and comprehensively, including monthly and quarterly progress notes and data analysis from ongoing treatment evaluations.

• Organize and facilitate training sessions for staff and team members to ensure effective implementation of treatment protocols and documentation procedures.

• Provide follow-up support through interviews and observational home visits as needed, ensuring continuous care and fostering long-term patient relationships.

• Act as a liaison to the broader mental health community, advocating for resources and support that align with the developmental goals of your clients.

• Keep meticulous records to guarantee the immediate availability of information to relevant stakeholders.

• Assist in the creation of training materials aimed at enhancing program effectiveness for patient care.

Patient Population / Client Focus\n

Qualifications\n

• A master's or doctoral degree in Counseling, Psychology, or a related field, with an emphasis on mental health therapy.

• An active and unrestricted Professional Counselor license in the state of Texas.

• A minimum of two years of clinical experience in counseling, with a preference for experience in outpatient settings.
