This position offers a full-time remote work arrangement allowing for flexibility while ensuring that our clients receive continuous care. The typical working hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs. Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, evaluating emotional, mental, and social factors impacting their well-being.

• Develop personalized care plans in collaboration with clients and their families, aligning with their specific goals and preferences.

• Provide counseling and emotional support to seniors and their families, helping them navigate complex transitions such as health crises or changes in living conditions.

• Advocate for clients' rights, ensuring access to community resources and services, and helping them navigate healthcare and legal systems.

Qualifications\n
