This position is fully remote, enabling the flexibility to set your own workspace while collaborating with colleagues through virtual meetings and telehealth sessions. As part of a community health clinic, you will engage with a variety of case management resources and participate in regular training designed to enhance service delivery and foster a collaborative workplace.

Key Responsibilities\n

• Maintain a manageable caseload of clients, ensuring regular check-ins to assess progress, stability, and the appropriate timing for service discharge.

• Build and nurture positive, professional relationships with clients, adhering to ethical counseling practices and maintaining confidentiality.

• Develop and implement personalized treatment plans in consultation with clients, continually monitoring their progress to adjust the plans as necessary.

• Conduct initial intake assessments to determine clients' needs and the most suitable therapeutic approach.

• Complete detailed documentation in a timely manner, including progress notes and treatment summaries, to support ongoing client care.

• Provide individual psychotherapy sessions that are tailored to the unique emotional and psychological needs of clients.

• Participate in ongoing supervision and peer consultation to ensure high-quality care and professional development.

• Collaborate effectively with other healthcare providers to create integrated care plans and enhance treatment outcomes.

Qualifications\n
