Key Responsibilities

• Conduct thorough evaluations by reviewing patient histories and performing specific assessments, resulting in tailored treatment plans based on evidence-based practices.
• Communicate evaluation findings and treatment plans clearly to patients and their families, fostering collaboration in goal-setting and treatment progress.
• Effectively implement treatment protocols, adapting interventions based on ongoing assessments and patient responses to therapy.
• Collaborate with other healthcare providers to ensure comprehensive patient care, making timely referrals as necessary and adjusting care plans based on updates in patient conditions.
• Oversee discharge planning and follow-up care while ensuring accurate and timely documentation of patient interactions, enabling clear communication within the care team.
• Manage your schedule effectively, meeting productivity targets and participating actively in marketing initiatives that enhance patient access to care.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy (DPT) from an accredited program.
• A valid Physical Therapy license in the State of New Jersey.

Preferred Experience
