Key Responsibilities

• Conduct thorough health assessments and screenings for residents, ensuring that all relevant medical histories and conditions are well-documented.
• Diagnose and manage both acute and chronic cardiology-related conditions, providing preventive care and engaging in health promotion initiatives.
• Implement tailored care plans based on community standards and individual patient needs.
• Collaborate closely with community organizations and healthcare stakeholders to design and facilitate health education programs focused on cardiovascular health.
• Ensure cultural sensitivity in all interactions while advocating for patients' rights and access to essential healthcare services.
• Coordinate referrals and manage follow-up care with other healthcare professionals to guarantee continuity of care.
• Maintain accurate, detailed medical records in compliance with all regulatory standards and organizational policies.
• Participate actively in community outreach events and health fairs aimed at increasing awareness of cardiology issues and providing necessary screenings.
• Stay informed on the latest community health trends, evidence-based practices, and relevant policies pertaining to cardiology.

Patient Population / Client Focus

The Nurse Practitioner in this role will primarily focus on providing care to residents of the Assisted Living Facility, many of whom may have complex medical histories that require specialized cardiology care. Your expertise will be vital in conducting health screenings and managing conditions that are prevalent among seniors.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• An active and unrestricted state nursing license.
• Current certification as a Nurse Practitioner, with a focus on cardiology being highly preferred.
• Strong clinical acumen with at least 3 years of relevant experience in cardiology or similar healthcare settings.
• Excellent interpersonal and communication skills to effectively collaborate with a diverse patient population.
• A demonstrated commitment to social justice principles and health equity.

Preferred Experience

Compensation and Benefits

The position offers a competitive salary range of USD 150,000 - 160,000 annually , commensurate with experience and qualifications. In addition, we provide a comprehensive benefits package, including medical, dental, and vision insurance, retirement plans, and paid time off.
