This is a full-time remote position based out of Gresham, Oregon, with your primary task being client interactions via telehealth platforms. As part of a community health clinic, you will work closely with a diverse team of healthcare professionals including psychologists, social workers, and occupational therapists. Key Responsibilities\n

• Conduct initial client assessments to identify their goals, preferences, and support systems, thereby laying the groundwork for tailored treatment strategies.

• Develop and continually refine individualized rehabilitation plans that align with client strengths, capabilities, and aspirations, ensuring they address evolving needs as treatment progresses.

• Collaborate with a multidisciplinary team to implement client-focused rehabilitation programs, ensuring a coordinated approach to each client's mental health care.

• Maintain accurate and detailed client records, documenting personal information, service delivery logs, insurance documentation, and communication notes that reflect client progress.

• Monitor ongoing client progress through regular evaluations, adjusting treatment plans and interventions as necessary to meet client goals effectively.

• Facilitate strong communication with clients, families, and relevant stakeholders to address barriers and reinforce support systems during job training and occupational placement programs.

• Prepare timely and insightful assessments and reports on client needs and progress, guaranteeing they receive the most effective care and interventions.

• Provide referrals and resources to clients as needed, guiding them toward additional support systems for their mental health and overall well-being.

You will primarily work with individuals facing various mental health concerns, including anxiety, depression, mood disorders, and trauma. Qualifications\n

• A Master’s degree in Counseling, Social Work, Psychology, or a related field is required.

• Current licensure as a Licensed Professional Counselor (LPC) in the state of Oregon.

• Demonstrated experience in mental health treatment, particularly with mood disorders and individuals in transitional phases, in both inpatient and outpatient settings.

• Proficient clinical skills in evidence-based practices including motivational interviewing, cognitive-behavioral therapy, and dialectical behavioral therapy.
