Key Responsibilities\n

• Conduct initial assessments with clients through virtual sessions, identifying their goals, preferences, and the most effective treatment options.

• Create and implement personalized rehabilitation strategies considering each client's unique educational background, skills, physical abilities, and future aspirations.

• Collaborate with our multidisciplinary team, which includes psychiatrists, occupational therapists, and psychologists, to design and oversee individualized treatment programs.

• Maintain comprehensive case management files detailing personal and insurance information, service history, and progress notes to ensure seamless care delivery.

• Regularly document and monitor client progress, adjusting therapeutic interventions as necessary to meet changing needs and support successful outcomes.

• Facilitate ongoing communication with clients as they participate in job training and placement programs, promptly addressing any challenges that arise.

• Engage clients’ family members for support and resolve issues effectively by leveraging community resources.

• Provide guidance and referrals for clients to access additional resources promptly when needed.

As a Licensed Professional Counselor at Divine Mind and Soul Therapy, LLC, you will primarily work with individuals experiencing a range of mental health issues, including anxiety, depression, mood disorders, and adjustment concerns. Qualifications\n

• A Master’s degree in counseling, social work, psychology, or a related field.

• Current licensure as a Licensed Professional Counselor (LPC).
