Lead Family Nurse Practitioner - Outpatient Care in Las Vegas

Work Environment

The FNP role is based onsite at our outpatient office, where you will work alongside a multidisciplinary team of healthcare professionals. Our facility is equipped with the latest technology and resources to support effective patient care. You will engage in a traditional work schedule from 9:00 a.m. to 6:00 p.m., with the occasional need for evening clinics until 8:00 p.m. In addition, participation in community outreach events on weekends may be required, ensuring that you are actively involved in promoting health and wellness initiatives outside the office hours.

Key Responsibilities

• Conduct thorough health assessments and screenings for a wide range of individuals and families, taking into account their unique health histories and socio-economic backgrounds.
• Diagnose, manage, and treat both acute and chronic medical conditions, with a focus on preventive care to enhance overall health and wellness.
• Collaborate closely with local community organizations and health stakeholders to design and implement innovative health education programs aimed at empowering patients.
• Provide culturally sensitive and patient-centered care, advocating for individual patient needs and rights to ensure equitable access to healthcare services.
• Facilitate referrals and follow-up care to ensure continuity and coordination throughout the treatment process.
• Maintain detailed and compliant medical records, staying up-to-date with regulatory requirements and best practices in health documentation.
• Actively participate in community outreach events and health fairs, delivering health screenings and raising awareness of health resources available to the community.
• Stay informed of emerging community health trends, evidence-based practices, and relevant public health policies to enhance quality of care.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, including underserved individuals and families from various cultural backgrounds. You will address their healthcare needs holistically, recognizing how social determinants of health impact their well-being. Qualifications

• A current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in Nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, ideally within community health or public health settings.
• Strong interpersonal and communication skills, enabling effective collaboration with patients and community partners.
• A passion for social justice and a commitment to advocating for vulnerable populations in need of healthcare services.

Preferred Experience

Compensation and Benefits
