Key Responsibilities\n

• Conduct comprehensive psychosocial assessments, utilizing interviews, observations, and medical record reviews to identify emotional, social, and environmental factors influencing patient recovery.

• Provide individual and family counseling sessions aimed at addressing adjustment issues, emotional wellness, and coping strategies during challenging times.

• Collaborate with the rehabilitation team to formulate individualized treatment plans that consider each patient’s psychosocial profile and rehabilitation goals.

• Assist patients in navigating community resources, such as support groups, vocational rehabilitation services, and housing assistance programs, to enhance their recovery experience.

• Educate patients and their families about available community resources and post-rehabilitation services, reinforcing their support system post-discharge.

• Actively participate in discharge planning meetings, facilitating transitions to home care or other settings and ensuring the continuity of care.

Compensation and Benefits\n

The compensation for this position ranges from USD 75,000 to 100,000 annually, based on experience and qualifications.
