Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through detailed interviews, insightful observations, and careful review of medical records to evaluate psychosocial factors influencing patient recovery.

• Deliver compassionate counseling and supportive interventions to patients and their families, effectively addressing challenges such as adjustment issues and emotional distress.

• Actively collaborate with members of the rehabilitation team to create individualized treatment plans that reflect the unique psychosocial needs of each patient, incorporating evidence-based practices.

• Guide patients in accessing vital community resources including support groups, vocational rehabilitation, and housing assistance, empowering them to achieve self-sufficiency.

• Regularly evaluate treatment effectiveness and collaborate on any modifications required to optimize patient recovery outcomes.

Qualifications\n

Compensation and Benefits\n
