Key Responsibilities\n

• Conduct comprehensive psychosocial assessments via virtual platforms to determine clients' strengths and needs.

• Deliver evidence-based therapy and counseling, including Cognitive Behavioral Therapy (CBT) and Dialectical Behavioral Therapy (DBT), to individuals, families, and groups.

• Collaborate with medical professionals, including physicians and psychiatrists, to coordinate integrated care plans for clients.

• Advocate for clients' rights, providing guidance to help them access necessary community resources and support services.

• Maintain accurate and timely documentation of client interactions, therapeutic progress, and outcomes in electronic health records.

Your work will primarily focus on clients at our Hospice Center, who may be facing terminal illness and related mental health challenges. Qualifications\n

• A Bachelor’s Degree in Social Work from an accredited program is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• A minimum of two (2) years of post-licensure experience in providing clinical social work services is essential.
