Key Responsibilities\n

• Perform in-depth psychosocial assessments, utilizing interviews, observations, and comprehensive reviews of medical histories to determine emotional, social, and environmental factors affecting patient recovery.

• Provide individualized counseling to patients and their families, addressing emotional hurdles, adjustment challenges, and overall well-being as they navigate their rehabilitation journeys.

• Collaborate closely with a multidisciplinary rehabilitation team to formulate and adapt individualized treatment plans that incorporate each patient’s psychosocial needs and goals.

• Guide patients in accessing valuable community resources, such as support groups, vocational rehabilitation programs, and housing assistance, to promote a seamless transition back to independent living.

• Engage actively in discharge planning processes, ensuring that all transitions, whether to home or to other care facilities, are executed smoothly and positively impact patient outcomes.

Patient Population / Client Focus\n

Our patient population is diverse, encompassing individuals recovering from a variety of conditions, including but not limited to strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n
