Key Responsibilities\n

• Conduct in-depth evaluations and assessments of clients' mental health and social needs, ensuring accurate documentation and analysis.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs), tailored to meet the unique needs of each child and family.
\n
• Monitor and evaluate client outcomes methodically, adjusting strategies and approaches in conjunction with the care team to promote optimal results.

• Provide support and guidance to families, offering them the necessary tools and resources to enhance the child's development and well-being.
\n
• Ensure compliance with all relevant Federal and State regulations, safeguarding the welfare of clients and upholding professional standards.
\n
• Build and maintain rapport with children and families, promoting a nurturing and therapeutic environment conducive to growth and healing.
\n
• Utilize evidence-based practices and follow Natural Learning Environment Practices to create effective joint plans for families.
\n
• Utilize a coaching model during sessions, focusing on natural environment teaching to optimize learning and engagement.

Patient Population / Client Focus\n

The LCSW will primarily work with young children who manifest developmental delays or disabilities. Our Children's Services program focuses on infants and toddlers, facilitating their growth across physical, emotional, cognitive, and communication domains. By collaborating with families and utilizing holistic approaches, the LCSW will play a pivotal role in aiding these children to reach their developmental milestones.

Qualifications\n

• Master's Degree or Doctorate in Social Work, recognized by the Department of Public Health.

• Licensure as a Clinical Social Worker (LCSW) is required.
