• Conduct comprehensive assessments of patients' mental and emotional well-being using interviews, conversations, and observational analysis to accurately assess individual needs.

• Formulate personalized treatment plans that may include therapeutic counseling, coordination of medication management, and referral to community resources.

• Facilitate individual and group therapy sessions, providing coping strategies and emotional support to help patients manage their experiences and feelings.

• Set specific, achievable goals with each patient regarding their treatment journey and continuously monitor progress.
\n
• Collaborate with a multidisciplinary team of healthcare professionals, including physicians and nurses, to ensure a coordinated approach to patient care.

• Provide emotional support to patients and their families during significant life transitions, including end-of-life planning.
\n
• Document all patient interactions thoroughly in our electronic health record system to ensure accurate records and facilitate ongoing care.

This role primarily focuses on individuals receiving hospice care, which may include patients facing terminal illnesses and their families. It is essential to approach each patient with sensitivity to their unique circumstances, offering a supportive presence that fosters trust and opens pathways for honest discussions about fears, hopes, and grief.

• A Master’s Degree in Social Work from an accredited institution.

• Current and valid LCSW license in the state of California.

• At least two years of relevant clinical experience in mental health settings, with a preference for experience in hospice or end-of-life care.

• Strong skills in therapeutic interventions tailored to diverse age groups and various behavioral health conditions.
