In this role, you will leverage your expertise as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW) to deliver critical case management and counseling services tailored to meet the biopsychosocial needs of our diverse patient population.

Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs and collaboratively create tailored treatment plans that address their specific requirements.

• Engage in empathetic counseling services that promote psychological healing and resilience, applying evidence-based practices to support short-term therapy and crisis intervention.

• Assist patients in navigating the complexities of healthcare paperwork, including entitlements, insurance claims, and benefits applications, ensuring they receive the support they need.

• Provide guidance to patients seeking assistance with prescription costs through available indigent programs and community resources, acting as a knowledgeable resource and advocate.

• Maintain detailed and accurate patient records, including progress notes and treatment plans, to ensure compliance with all regulatory standards and contribute to clinical outcomes.

• Coordinate transportation assistance for patients when necessary to facilitate access to care, demonstrating a commitment to overcoming barriers to treatment.

• Actively participate in clinical and team meetings, contributing insights and updates regarding patient progress and treatment adaptations.

Patient Population / Client Focus\n

The clients you’ll serve encompass a broad range of ages and backgrounds, including adolescents, adults, and families dealing with various mental health challenges. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling from an accredited institution.

• A current and valid independent license to practice therapy in Oregon, such as LPC, LCSW, or LMFT.

• A minimum of two (2) years of direct clinical experience providing mental health counseling or therapy services is required.

Preferred Experience\n

• Experience with diverse populations and an understanding of multicultural counseling techniques.

• Familiarity with telehealth practices and electronic medical records systems.

• A background in crisis intervention, with the ability to respond effectively to acute emotional distress.

Compensation and Benefits\n
