Physical Therapist

We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room to provide skilled nursing care, case management, and hospice care to patients. The role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team to ensure quality care. Strong communication and teamwork skills are essential. A minimum of two years of relevant experience is required. Responsibilities:

• Conduct patient assessments and monitor health status.
• Administer medications and treatments as prescribed.
• Develop and implement individualized patient care plans.
• Collaborate with interdisciplinary healthcare team members.
• Provide case management and hospice care services.
• Maintain accurate and timely patient documentation.
• Educate patients and families regarding care plans and health management.

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of two years of hospital, home health, or hospice nursing experience.
• Clinical assessment, medication administration, and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Driver's license, car insurance, and automobile for field work.

• Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
• Additional certifications relevant to specialty areas.

• Ability to work effectively in fast-paced and dynamic healthcare environments.
• May require on-call availability depending on facility needs.
• Work involves caring for patients with advancing age, frailty, and serious illness.

• Day shift schedule, typically Monday through Friday.
• Shift length may vary between 8 to 10 hours per day.
• Shift assignments based on facility requirements and patient care needs.
