Key Responsibilities\n

• Conduct comprehensive assessments of clients to determine mental health needs and tailor individualized treatment plans.

• Foster effective communication with physicians and other health professionals to report on patient progress, address concerns, and identify issues that require immediate attention.

• Maintain detailed and accurate clinical records and progress notes in compliance with organizational policies, ensuring confidentiality and adherence to relevant regulations.

• Collaborate closely with multidisciplinary healthcare teams in formulating treatment strategies and discharge plans that align with psychosocial assessments and evidence-based interventions.

• Provide effective counseling and crisis intervention services for patients, addressing emotional and behavioral issues that may impact their daily life.

• Assist clients with discharge planning and care coordination, connecting them with appropriate services and resources in the community to support their ongoing mental health needs.

• Engage with clients’ family members and support systems to facilitate comprehensive treatment approaches and discharge planning.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at Hollow Haven Counselling, you will work with a diverse patient population, encompassing children, adolescents, and adults facing a range of mental health challenges. Qualifications\n

• A Master's degree in Social Work, Counseling, or Marriage and Family Therapy from an accredited institution.

• Licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC) is required.

• A minimum of 3 years of direct experience within an outpatient setting, providing mental health counseling to a varied clientele.
