Key Responsibilities\n

• Maintain open lines of communication with physicians and healthcare providers to report on patient progress, concerns, and potential issues.

• Consult and coordinate with clients’ family members or support systems regarding treatment plans and discharge planning to ensure comprehensive care.

• Meticulously document clinical and progress notes in a timely manner, adhering to departmental policies and best practices.

• Collaborate with healthcare team members to craft individualized treatment plans that are informed by thorough psychosocial assessments and apply evidence-based interventions.

• Assist clients with discharge planning, focusing on facilitating streamlined access to necessary services within the region.

• Provide individual and group therapy sessions, as well as substance abuse assessments, employing a recovery-oriented framework.

Patient Population / Client Focus\n

In this role, you will work with a diverse client population, including adults facing various mental health challenges such as anxiety disorders, depression, trauma, substance use disorders, and relationship issues. Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology is required.

Compensation and Benefits\n
