Key Responsibilities\n

• Conduct thorough psychosocial assessments through patient interviews, observations, and review of relevant medical records to understand emotional, social, and environmental factors affecting recovery.

• Provide compassionate counseling and ongoing support to patients and their families, facilitating adjustment to rehabilitation and addressing emotional challenges.

• Collaborate closely with interdisciplinary rehabilitation teams to create individualized treatment plans that encompass clients' psychosocial needs alongside their physical rehabilitation goals.

• Assist patients in identifying and accessing community resources, such as support groups and housing assistance, to foster their independence.

• Educate patients and their families about available community resources and post-rehabilitation services to ensure effective transition and ongoing support after discharge.

• Participate in discharge planning to ensure a smooth transition for patients into their home or community settings post-rehabilitation.

Patient Population / Client Focus\n

As an LCSW at American Counseling Center, you will work with a diverse population that includes individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

• A Master’s degree in Social Work (MSW) and possession of a valid state licensure as a Clinical Social Worker (LCSW).
