Key Responsibilities

• Conduct thorough health assessments and screenings for residents, ensuring timely and appropriate intervention for both acute and chronic conditions.
• Diagnose and manage a range of medical issues, coordinating care plans that include preventive strategies and health promotion tailored to individual patient needs.
• Participate actively in rounds with Cardiac procedural patients, ensuring continuity of care and addressing any emerging concerns immediately.
• Document medical histories and maintain detailed patient records in accordance with regulatory standards and best practices, using electronic health record (EHR) systems effectively.
• Engage with community organizations, spearheading health education initiatives aimed at promoting awareness around cardiac health.
• Provide culturally sensitive care, advocating for patients' rights and facilitating access to needed healthcare resources.
• Organize and lead community outreach events and health fairs, delivering screenings and vital health education to empower community members.
• Stay abreast of emerging health trends and evidence-based practices in cardiology to continuously improve care strategies and methodologies.

Patient Population / Client Focus

Our focus is on a diverse population residing within the Assisted Living Facility, including elderly patients with complex cardiac conditions. Many residents may have coexisting medical issues that require careful management and a team-based approach. You will also work with caregivers and families, educating them on best practices for ongoing cardiac care.

Qualifications

• Must possess a valid Nurse Practitioner credential.
• Current State Nursing Licensure in New York is required.
• A Master's degree in nursing from an accredited program is essential.
• Strong commitment to social justice and health equity, along with advocacy for underserved populations.

Preferred Experience

• A minimum of two years of clinical experience in a Cardiology setting is required.
• Proficient in establishing rapport and working effectively with diverse patient populations, demonstrating excellent interpersonal and communication skills.

Compensation and Benefits
