Position Overview

Atlantis Athletes Physical Therapy Care is seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team in Saratoga Springs, New York. Work Environment

The role is predominantly onsite at our outpatient office, where you will be an integral member of a collaborative care team. This multidisciplinary team includes a physician, other nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, and registered nurses who all work together to optimize care delivery. Key Responsibilities

• Deliver outstanding primary care services, including diagnosis and treatment of acute and chronic health conditions.
• Conduct thorough office visits, utilizing evidence-based practices to inform clinical decisions.
• Administer annual wellness visits and health risk assessments, ensuring comprehensive documentation that captures the holistic health status of each patient.
• Coordinate care with physicians, specialists, and support agencies to streamline patient management and enhance health outcomes.
• Lead initiatives related to population health, ensuring compliance with preventive care guidelines, such as having all eligible patients receive critical screenings.
• Provide timely phone triage and outreach to patients, ensuring their concerns are addressed promptly.
• Educate patients about their health conditions, treatment plans, and preventative measures, reinforcing their understanding and engagement.
• Participate in community outreach and promotional events, representing our commitment to health education.
• Conduct home visits when necessary to deliver care to patients with mobility challenges or other barriers to visiting the clinic.
• Collaborate closely with the Medical Director and operational leadership to improve clinic processes and patient care delivery.

Patient Population / Client Focus

Qualifications

• Master's Degree in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• National board certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner, obtained from AANP or ANCC.
• Active and unrestricted Nurse Practitioner license in the state of New York.
• Possession of a current DEA license.
• A minimum of one year of experience in primary care, internal medicine, geriatrics, or family medicine is strongly preferred.
• Proficient understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and conducting health risk assessments.
• A strong commitment to patient education, disease prevention, and team collaboration.

Preferred Experience

• Experience working with Medicare patients, particularly in an outpatient setting.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• Background in patient advocacy and community health initiatives.

Compensation and Benefits

The annual salary for this position ranges from USD 120,000 to 160,000, commensurate with experience and qualifications.
