Full-Time Nurse Practitioner for Pain Management in Assisted Living Facility

Work Environment

The Nurse Practitioner will work onsite at our Assisted Living Facility, which is designed to provide a warm and nurturing atmosphere to our patients. You will be part of an interdisciplinary team that includes doctors, nurses, and allied health professionals. The facility is organized to promote collaboration and communication among healthcare providers, which is essential for delivering optimal care. Your regular working hours will be Monday to Friday, 40 hours per week, with occasional Saturday coverage required. This structured work schedule allows for a balanced professional and personal life.

Key Responsibilities

• Perform comprehensive assessments of physical and psychological health to manage complex patient profiles effectively.
• Identify normal and abnormal clinical findings and document them accurately for continued care.
• Develop, implement, and adjust individualized treatment plans based on patient response and evolving needs.
• Evaluate and monitor the physical and emotional responses of patients to pain management interventions and adjust accordingly.
• Prescribe necessary medications and treatment modalities, including immunizations following CDC guidelines.
• Maintain open and effective communication with physicians and other healthcare providers, facilitating interdisciplinary treatment approaches.
• Initiate and regularly update the Problem/Medication list during each patient encounter, ensuring an integrated care pathway.
• Ensure completeness, accuracy, and timeliness of all patient documentation to meet health regulatory standards.

Patient Population / Client Focus

This role specifically focuses on managing the healthcare needs of elderly patients in the Assisted Living Facility. Patients often present with complex chronic pain issues and comorbidities that necessitate a comprehensive, multifaceted approach to pain management. The Nurse Practitioner will pursue strategies tailored to enhance each patient's quality of life, ensuring compassionate and respectful care that aligns with their unique preferences and needs.

Qualifications

• A Master’s degree in Nursing from an accredited Nurse Practitioner program.
• A current Registered Nurse (RN) License in New York State.
• Basic Life Support (BLS) Certification.
• Strong clinical judgment and critical thinking skills are essential.
• Outstanding interpersonal and communication skills to facilitate collaboration within the healthcare team.

Preferred Experience

• Minimum of 2-3 years of clinical experience as a Nurse Practitioner, with a preference for those with a background in cardiology or pain management.
• Experience in working with elderly populations and an understanding of their specific healthcare needs.

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience and qualifications.
