Full-Time Nurse Practitioner at Pulse Point Internal Medicine - Assisted Living Facility

Position Overview

Pulse Point Internal Medicine is seeking a dedicated and compassionate Nurse Practitioner to join our team at our Assisted Living Facility in picturesque Saratoga Springs, New York. This full-time role offers the opportunity to work closely with a diverse patient population, primarily focusing on older adults enrolled in Medicare, ensuring they receive the highest standard of primary care while fostering meaningful patient-provider relationships.

Work Environment

This position will be predominantly onsite within our Assisted Living Facility, working as part of a collaborative care team. Your colleagues will include a physician, other nurse practitioners or physician assistants, medical assistants, dedicated medical scribes, registered nurses, and care managers. Key Responsibilities

• Deliver high-quality primary care to patients, conducting independent office visits for both routine check-ups and acute health issues.
• Perform annual wellness visits and thorough health risk assessments, ensuring complete and accurate documentation of each patient’s health status.
• Coordinate care and collaborate effectively with other healthcare providers, specialists, and community testing facilities.
• Lead initiatives in population health, ensuring that all eligible patients receive recommended screenings and preventive health measures in a timely manner.
• Engage in phone triage and outreach efforts to provide timely support and follow-up care.
• Educate and empower patients about their health conditions, care plans, and treatment options.
• Participate in community health promotion activities to raise awareness and support for health initiatives.
• Conduct home visits when necessary, providing personalized care to enhance patient outcomes.
• Work closely with the Medical Director and operational leadership to support clinic management and quality improvement initiatives.

Patient Population / Client Focus

Your primary focus will be on older adults receiving Medicare, with an emphasis on preventive care and chronic disease management. You will play a vital role in guiding patients through their health journeys, ensuring they understand their care plans and making informed decisions about their health. Building trusting relationships is critical, as it enhances patient engagement and promotes adherence to treatment.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or a graduate of an NP Certification Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized boards such as AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Current DEA license for prescribing medications.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Strong knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments.
• A collaborative mindset and a passion for team-based healthcare delivery.
• Bilingual proficiency is a plus.

Preferred Experience

• Prior experience working in an Assisted Living Facility or similar setting.
• Demonstrated involvement in population health initiatives or quality improvement projects.
• Experience with electronic medical record (EMR) systems tailored for primary care.

Compensation and Benefits

The annual salary for this position ranges from $180,000 to $200,000, commensurate with experience and expertise. In addition to a competitive salary, we offer:
