Work Environment

The Family Nurse Practitioner position is office-based, allowing for direct interaction with patients in a welcoming clinic environment. Our work culture is collaborative, supportive, and deeply invested in the community's well-being. Regular work hours are 9:00 a.m. to 6:00 p.m., with some evening clinics extending until 8:00 p.m. Participation in weekend community outreach events, such as health fairs, is also encouraged as part of our commitment to broadening access to care.

Key Responsibilities

• Conduct comprehensive health evaluations, identifying and screening for physical, mental, and emotional health needs among individuals and families.
• Diagnose and create treatment plans for acute and chronic conditions, offering preventive health care and education on lifestyle modifications.
• Develop and implement health education programs tailored to community needs, working closely with local organizations.
• Provide culturally sensitive and compassionate care, ensuring that patients' rights are upheld while actively advocating for better healthcare access.
• Facilitate referrals and coordinate follow-up care, ensuring continuity in the treatment process for all patients.
• Document all patient interactions in an accurate and timely manner, adhering to regulatory compliance requirements.
• Stay informed about evolving community health trends and best practices to continuously improve care delivery.

Patient Population / Client Focus

You will primarily serve a diverse patient population encompassing families and individuals from various backgrounds, many of whom may face socioeconomic barriers to health access. Your role is essential in identifying unique challenges and offering tailored solutions that promote health equity.

Qualifications

• Possession of current Nurse Practitioner certification.
• Valid and active state nursing license in New York.
• A Master's degree in nursing from an accredited institution.
• At least three years of clinical experience, ideally within community health or public health sectors.
• Excellent interpersonal and communication skills, enabling effective collaboration with patients from diverse backgrounds.
• Strong commitment to social justice and health equity initiatives, with a passion for advocating for underserved populations.

Preferred Experience

• Experience with community health initiatives and public health programming.
• Familiarity with electronic health record (EHR) systems for efficient documentation and patient management.
• Background in family medicine, pediatrics, or gerontology may be advantageous.

Compensation and Benefits

• Annual salary range of USD 180,000 – 200,000, which will be determined based on experience and qualifications.
