SLI Rehab & Physical Therapy is actively seeking a dedicated Family Nurse Practitioner (FNP) to join our team in Saratoga Springs, New York. Work Environment

This position is based onsite at various schools in Saratoga Springs, allowing you to directly impact the health and wellness of children and families. Your work schedule will typically range from 9:00 a.m. to 6:00 p.m., with some evening clinics extending to 8:00 p.m. Participation in weekend community outreach events and health fairs will be required from time to time, fostering relationships and health awareness in the local community. An average workweek consists of approximately 40 hours, enabling a well-rounded work-life balance.

Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, identifying health risks and concerns.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health education tailored to the student population.
• Design and deliver educational sessions in collaboration with local stakeholders to foster health literacy and address common health issues affecting the community.
• Advocate for patients' rights and access to healthcare services, ensuring culturally sensitive care tailored to diverse backgrounds.
• Coordinate referrals and follow-up care with other healthcare providers to guarantee continuity and quality of care.
• Maintain accurate and detailed medical records to meet regulatory standards and enhance care continuity.
• Engage in community outreach initiatives and health fairs, providing essential health screenings and resources to underserved populations.
• Stay informed on community health trends, evidence-based practices, and relevant local health policies to ensure high standards of care.

Patient Population / Client Focus

Your primary focus will be on children, students, and their families within the Saratoga Springs community. Understanding their unique challenges and healthcare needs will be essential in delivering effective, culturally competent care. Qualifications

• Current certification as a Nurse Practitioner.
• An active state nursing license in New York.
• A Master's degree in nursing from an accredited program.

Preferred Experience

Compensation and Benefits
