Work Environment

This position is primarily onsite, where you will provide in-home healthcare services, allowing you to build close relationships with your patients. Key Responsibilities

As an Internal Medicine NP/PA, your responsibilities will include:

• Conducting thorough assessments of patients’ health by performing detailed physical examinations and reviewing medical histories.
• Developing and implementing comprehensive management plans tailored to individual patient needs, ensuring both immediate and long-term care considerations.
• Collaborating with physicians to address urgent patient care issues, while also facilitating preventive care and chronic disease management.
• Educating patients and their families about health conditions, treatment options, and lifestyle modifications that contribute to better health outcomes.
• Maintaining accurate and up-to-date electronic health records (EHR) to support coordinated care efforts.
• Managing routine follow-ups and monitoring patient progress to assess the efficacy of treatment plans.
• Participating in team meetings to discuss patient cases and develop collective care strategies.

Patient Population / Client Focus

This role predominantly serves adults in the Saratoga Springs area who require in-home medical care related to various internal medicine conditions. You will encounter a diverse population, including elderly patients and individuals with chronic illnesses, necessitating a nuanced understanding of medical, social, and psychological factors influencing health. Building rapport and trust with patients is essential to effectively manage their healthcare journeys.

Qualifications

• A Bachelor's Degree in Nursing or a related discipline is required.
• Completion of an accredited Nurse Practitioner or Physician Assistant program.
• Current New York state PA/NP license is mandatory.
• Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is preferred.

Preferred Experience

• Minimum of 2 years of experience as a Nurse Practitioner or Physician Assistant, preferably in an ambulatory care setting.
• Experience in managing patients with chronic diseases, elderly care, or home care settings is highly desirable.
• Familiarity with electronic health records (EHR) and telehealth technologies is advantageous.

Compensation and Benefits
