Little Vista Health Group is seeking a full-time Family Nurse Practitioner to join our dedicated team in Saratoga Springs, New York. This pivotal role involves providing holistic healthcare services to individuals and families within the community, particularly focusing on the needs of residents in our Assisted Living Facility. Work Environment

This role is primarily onsite at our Assisted Living Facility in Saratoga Springs, NY, a vibrant area known for its rich history and community spirit. The facility is designed to cater to residents who require assistance with daily living activities while also promoting independence and wellness. Your work schedule will typically be from 9:00 a.m. to 6:00 p.m., with occasional clinics extending until 8:00 p.m. Participation in community outreach events or health fairs during weekends may also be part of your responsibilities, ensuring that we reach a broader audience in the community.

Key Responsibilities

• Conduct thorough health assessments for residents and other individuals in the community, utilizing evidence-based practices to inform your evaluations.
• Diagnose and manage a wide range of medical conditions, ensuring that preventive care measures are prioritized and integrated into each patient’s healthcare plan.
• Design and implement health education initiatives in collaboration with community organizations, focusing on preventative health practices and chronic disease management.
• Advocate for patients’ needs, ensuring culturally sensitive care that respects diverse backgrounds and healthcare beliefs.
• Coordinate referrals and facilitate follow-up care to maintain continuity and quality of service for each patient.
• Maintain accurate and detailed medical records, compliant with regulatory standards and best practices in patient confidentiality.
• Engage in community outreach activities to foster relationships and raise awareness of health services available.
• Stay informed about community health trends and participate in professional development initiatives to enhance your clinical skills.

Patient Population / Client Focus

This position primarily focuses on residents of the Assisted Living Facility, many of whom experience complex health challenges related to aging. You will address diverse health issues, providing care that ranges from routine check-ups to management of chronic conditions and rehabilitation efforts. Qualifications

• Current certification as a Family Nurse Practitioner is required.
• An active state nursing license is essential to practice in New York.
• A Master’s degree in Nursing from an accredited program.
• A minimum of 3 years of clinical experience is mandatory, with a preference for experience in community health or public health settings.

Compensation and Benefits

We offer a competitive salary range of $180,000 - $200,000 per year, commensurate with experience and qualifications.
