Work Environment

This Nurse Practitioner role is performed onsite and involves visiting patients in their homes, allowing for an intimate and comfortable setting that fosters trust and open communication. Key Responsibilities

• Conduct thorough initial patient screenings, including taking patient history and performing physical examinations, to establish comprehensive care plans.
• Ensure up-to-date and accurate documentation in patient records, verifying details at each appointment to maintain compliance with healthcare standards.
• Analyze laboratory test results and diagnostic imaging to inform treatment decisions, determining the need for follow-up appointments or referrals.
• Deliver high-quality care while achieving and maintaining company metrics related to productivity and patient satisfaction.
• Act as a primary resource for patients, offering ongoing educational support, counseling, and guidance regarding chronic and complex health issues.
• Educate patients on health maintenance, prevention strategies, and treatment plans while emphasizing harm reduction techniques specific to their conditions.

Patient Population / Client Focus

Qualifications

• Master's Degree in Nursing from an accredited institution.
• Current New York State Nurse Practitioner (NP) licensure.

Compensation and Benefits

This full-time position offers a competitive salary ranging from $150,000 to $160,000 annually, in addition to a comprehensive benefits package that includes medical, dental, and vision coverage, retirement plan options, and paid time off.
