Brightwood Orthopedic and Family Medicine Care is seeking a dedicated and compassionate Family Nurse Practitioner to join our dynamic healthcare team in Saratoga Springs, New York. Work Environment

You will be working onsite in a collaborative hospital environment alongside a multidisciplinary care team. This includes physicians, nurse practitioners, physician assistants, medical assistants, registered nurses, and care managers, all dedicated to improving patient outcomes. Our department emphasizes care coordination and values quality over volume, enabling our team to focus on the provision of exceptional primary care tailored to each patient's needs.

Key Responsibilities

• Provide high-quality primary care services, including office visits for routine check-ups and acute health issues.
• Conduct annual wellness visits and health risk assessments with a comprehensive approach to holistic care and accurate documentation.
• Enhance patient care through proactive care coordination with specialists, testing facilities, and other healthcare agencies.
• Assume leadership in our population health initiatives, ensuring all eligible patients receive critical preventive screenings and assessments.
• Assist with phone triage and patient outreach to enhance care accessibility and responsiveness.
• Educate and empower patients regarding their health conditions, treatments, and individualized care strategies.
• Account for other care-related responsibilities as assigned and report directly to the Medical Director while collaborating closely with operational leadership.

Patient Population / Client Focus

Your primary focus will be on a diverse population of older adults enrolled in Medicare. Our innovative care model is centered on the community you serve, and you will play a crucial role in ensuring these patients receive personalized and effective care. By cultivating meaningful patient-provider relationships, you will help patients feel connected and engaged with their healthcare journey.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or graduate from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in primary care settings including internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Preferred Experience

• A proven passion for teamwork and collaborative care approaches.
• Desire to engage with an innovative healthcare model focused on population health strategies.
• Bilingual proficiency in relevant languages is a plus.

Compensation and Benefits

We offer a competitive salary range of $115,000 to $150,000 annually, commensurate with experience and expertise. In addition to a generous base salary, our benefits package includes:
