Key Responsibilities

• Conduct comprehensive physical assessments to design tailored therapy plans that address each patient's unique needs.
• Develop and implement individualized, progressive therapy programs focused on improving mobility, managing pain levels, enhancing balance, and overcoming functional limitations.
• Provide hands-on treatments, which include therapeutic exercises, gait training, and strategies for fall prevention.
• Guide patients in safely regaining and maintaining autonomy in their activities of daily living, especially within the rehabilitation context.
• Deliver educational support to patients, their families, and facility personnel regarding exercises, care practices, and wellness strategies to foster a comprehensive approach to rehabilitation.
• Maintain accurate and up-to-date patient records that meet healthcare compliance standards and support continuity of care.
• Engage in ongoing monitoring of patient progress, modify treatment plans as necessary, and effectively communicate any changes in conditions to the appropriate medical staff and families.
• Collaborate with nurses, caregivers, and other staff to ensure synchronized and thorough patient care delivery.

Qualifications

• An active New Jersey Physical Therapy license is required.
• A Bachelor's Degree in Physical Therapy; a Doctorate or Master's Degree from an accredited program is preferred.
• A minimum of 2 years of physical therapy experience is mandated, particularly in geriatric or assisted living contexts.
• Strong clinical reasoning abilities, exceptional communication skills, and a collaborative spirit are essential for success in this role.
