Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify emotional, social, and environmental challenges that impact patient care and family dynamics.

• Provide tailored interventions, including crisis management and therapeutic counseling, to support patients and families coping with serious health issues.

• Facilitate effective discharge planning through collaboration with medical staff, ensuring that the transition from hospital to home or hospice care is seamless and supportive.

• Utilize a variety of therapeutic modalities, including individual, group, and family therapy, to address unique patient needs.
\n
• Educate patients and families on coping strategies and available resources to help them adapt to their life circumstances.

• Engage with oncology patients to assess mental health needs, addressing concerns such as anxiety and depression with appropriate counseling.

• Participate in on-call rotations to offer after-hours support to patients and assist in urgent situations.
\n
• Act as a consultant within the interdisciplinary team, sharing expertise in social work to enhance overall patient care outcomes.

• Contribute to community services, outreach programs, and educational sessions that promote awareness and support for cancer survivors and their families.

• Present findings and insights at educational conferences to enhance the knowledge base within psychosocial oncology.

Patient Population / Client Focus\n

Your primary focus will be on individuals facing terminal illnesses, particularly those receiving hospice care. You will work closely with patients and their families to help them process their emotions, adapt to changing health conditions, and explore available resources to improve their quality of life. This role is pivotal in helping clients find peace, enhance their coping mechanisms, and make informed choices that align with their values and wishes.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Licensed as an Independent Clinical Social Worker (LICSW) in the state of Washington.

• Strong understanding of psychosocial oncology and experience with hospice or end-of-life care is preferred.

• Excellent interpersonal and communication skills, with the ability to empathize and build rapport with diverse populations.

• Proven ability to collaborate effectively within a healthcare team and engage multidisciplinary partners in care delivery.

Compensation and Benefits\n

The annual salary for this position ranges from $70,000 to $100,000, commensurate with experience and qualifications.
