Key Responsibilities\n

• Collaborate closely with primary care practitioners to identify, treat, and manage patients' mental health and psychosocial issues, enhancing the overall effectiveness of the healthcare team.

• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations to inform treatment objectives and recommendations.

• Provide immediate mental health interventions to patients in crisis, ensuring timely access to care.

• Advise primary care teams on cases requiring specialized mental health services or referrals, facilitating seamless patient transitions within the healthcare system.

• Regularly monitor patient progress through follow-up evaluations, adapting treatment plans as necessary to ensure positive outcomes.

• Develop and implement relapse prevention strategies in collaboration with patients, empowering them toward sustainable recovery.

• Engage with patients at risk for deteriorating health outcomes and work on preventive measures tailored to their specific needs.

• Supervise the delivery of mental health services within the primary care setting and, with patient consent, coordinate with external treatment providers.

• Document all patient interactions and progress meticulously, ensuring comprehensive patient records that support ongoing treatment and compliance.

• Maintain continuous communication with primary care providers to keep them informed about patients’ mental health statuses, contributing to collaborative treatment planning.

• Deliver behavioral interventions aimed at promoting healthier lifestyle choices among patients with physical health challenges.

• Empower patients through educational resources and self-management skills, enhancing their engagement in their recovery journey.

• Consult with management and peers regarding behavioral health services, identifying opportunities for program evaluation and development.

• Assist patients in adhering to medical recommendations made by healthcare practitioners, including addressing any side effects of medications.

• Uphold the Clinical Professionals' Code of Ethics and take on additional responsibilities as required.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• A valid Licensed Professional Counselor (LPC) license is mandatory.

• Familiarity with developmental psychopathology and various therapy approaches, including individual, family, and group therapy.

Compensation and Benefits\n

The starting salary for this role ranges from USD 90,000 to 105,000 yearly, commensurate with experience and qualifications.
