Key Responsibilities\n

As a Licensed Mental Health Counselor, your daily activities will include, but are not limited to:

• Utilizing evidence-based patient education programs and established social work methodologies, you will assess and develop individualized care plans to address the psychological and emotional needs of your clients.

• Providing supportive counseling services tailored to each patient's unique situation, focusing on mental health and emotional well-being.

• Facilitating adjustment to treatment modalities, such as dialysis, by monitoring patients’ psychosocial status and implementing interventions to promote optimal mental health.

• Leading goal-directed counseling sessions for patients who are pursuing transplant options, offering educational resources and emotional support throughout the process.

• Connecting patients to community resources, such as home health services and vocational rehabilitation programs, to enhance treatment outcomes and facilitate recovery.

• Conducting assessments regarding advance directives and assisting with discussions among the patient, healthcare team, and family to ensure that patient wishes are respected.

• Participating in interdisciplinary team meetings to evaluate patient progress and adjust care plans as necessary, ensuring all aspects of the patient’s health are addressed.

Qualifications\n

To be successful in this role, candidates must meet the following qualifications:

Compensation and Benefits\n
