This full-time position offers the flexibility of a remote work setting, allowing you to manage your schedule while still providing crucial support to our clients. While you will primarily work from home, occasional home and community visits may be required to ensure comprehensive care for your caseload. Key Responsibilities\n

• Deliver intensive case management services to a small caseload of children and families, providing tailored support and intervention.

• Triage and prioritize incoming care needs, ensuring timely response and allocation of resources.

• Assist families in completing necessary forms and applications, enhancing access to critical resources.
\n
• Engage with families to identify and address social determinants of health (SDOH) that may affect mental well-being.

• Facilitate connections for families seeking community resources and support systems, enhancing their overall resilience.

• Monitor and track referrals, providing follow-up to ensure all necessary services are delivered effectively.
\n
• Help patients schedule and attend various appointments, including medical, behavioral health, and specialist consultations.
\n
• Provide administrative support, including records retrieval, eligibility screening, and goal tracking.
\n
• Conduct home and community visits when necessary to assess and support patients in their environments.
\n
• Collaborate with an interdisciplinary care team that includes behavioral health professionals, medical staff, and external partners to coordinate holistic patient care.

• Document all patient interactions and services accurately using electronic health records and secondary databases.
\n
• Participate actively in internal and external meetings, contributing to case conferencing and administrative discussions.

• Engage in continued education and training on health coaching, patient engagement techniques, and best practices in social work.

• Submit timely and accurate reports and data as requested by management, contributing to quality improvement initiatives.

• Provide insights and feedback regarding case management effectiveness and potential areas for enhancement.

Patient Population / Client Focus\n

Your primary focus will be on children and families who are navigating complex mental health and social service landscapes. These clients may be dealing with trauma, behavioral issues, and various socioeconomic challenges. By working closely with these individuals, you will help them develop resilience, access appropriate services, and ultimately improve their quality of life.

Qualifications\n

• Must hold an active LCSW license in the state of Pennsylvania.

• A Master’s degree in Social Work (MSW) from an accredited program is required.
\n
• At least 2 years of clinical experience in mental health or social service settings, demonstrating the ability to work effectively with diverse populations.

• Proficient in utilizing evidence-based therapeutic interventions, such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Eye Movement Desensitization and Reprocessing (EMDR).

• Strong skills in case management, mental health assessment, and crisis intervention.
\n
• Excellent communication and interpersonal skills, allowing for effective collaboration with clients and team members.

• Demonstrated ability to navigate community resources and services to benefit patients.

Compensation and Benefits\n

The salary for this position ranges from $80,000 to $90,000 annually, depending on experience and qualifications.
