Key Responsibilities\n

• Develop a tentative schedule of appointments, home visits, and assessments for clients, strategically designed to support ongoing client needs and treatment objectives.

• Coordinate home visits to conduct in-home observations, chart reviews, and staff interviews, gathering comprehensive data to guide clinical decision-making.

• Monitor the implementation of individual program plans to ensure effectiveness and adherence to clinical standards.

• Participate actively in Interdisciplinary Team Meetings, offering insights and collaborating with fellow healthcare professionals to enhance patient care.

• Complete detailed monthly and quarterly progress notes, ensuring accurate documentation of patient milestones and treatment responses.
