Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to effectively identify and meet the needs of clients.

• Actively participate in dialogue with supervisors, family members, and relevant stakeholders to communicate an individual’s progress and challenges.

• Develop and provide tentative scheduling for appointments and home visits to monitor and support client needs.
\n
• Coordinate and participate in home visits to perform in-home observations, chart reviews, staff interviews, and data analysis for comprehensive treatment planning.

• Ensure the timely and accurate implementation of program plans, thereby enhancing the therapeutic outcomes for patients.
\n
• Engage in interdisciplinary team meetings to collaborate on case management and treatment strategy adjustments.
\n
• Prepare monthly and quarterly documentation of client progress notes to evaluate treatment efficacy.
\n
• Facilitate training and certification sessions to ensure all team members are competent in program plan implementation and documentation procedures.
\n
• Provide continuous follow-up through interviews and observations, ensuring clients receive ongoing support tailored to their evolving needs.

• Maintain meticulous records to provide transparency and accessibility of information to all relevant parties.

• Act as a liaison between clients and the larger mental health community to foster deeper support networks for individuals.

• Assist in the development of training materials to enhance programs tailored to individual client needs.
