Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and provide recommendations to primary care providers regarding individualized treatment goals and plans.

• Deliver brief, focused therapeutic interventions to clients experiencing mental health challenges, ensuring they receive timely support.

• Work collaboratively with primary care providers to identify patients best suited for continued care within the primary health setting versus those requiring specialized mental health facilities.

• Initiate proactive follow-up care for clients, assessing progress and adjusting treatment plans as necessary to promote optimal outcomes.

• Develop comprehensive relapse prevention plans for clients to help them maintain stable functioning and provide resources for ongoing support.
\n
• Document patient progress meticulously, ensuring that treatment charts are accurate and up-to-date, facilitating effective communication with interdisciplinary teams.

• Educate patients on self-management techniques and provide strategies for coping with any side effects of prescribed medications initiated by primary care providers.

Patient Population / Client Focus\n

Our patient population primarily consists of diverse families and children facing a wide array of mental health issues. As a Licensed Clinical Professional Counselor, you will work closely with individuals experiencing emotional distress, developmental concerns, or psychosocial challenges. Your role will be pivotal in ensuring that clients feel understood and equipped with the skills needed for recovery.

Qualifications\n

• A Master’s Degree in Clinical Counseling, preferably from an accredited institution.

• A current Licensed Clinical Professional Counselor (LCPC) credential.

• Experience working within a federatively qualified health center (FQHC) and knowledge of Medicaid and Medicare practices are highly valued.

Compensation and Benefits\n

The salary range for this full-time position is competitive, with an annual payout between USD 87,000 and 105,000, commensurate with experience and qualifications.
