Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through in-depth interviews, observations, and thorough reviews of medical records to evaluate the emotional, social, and environmental factors affecting patient recovery.

• Deliver empathetic counseling and emotional support to patients and their families, especially in addressing adjustment issues related to recovery and coping strategies during challenging times.

• Work collaboratively with our rehabilitation team to craft individualized treatment plans that address the psychosocial needs of patients, integrating approaches that align with their rehabilitation goals.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, fostering their road to independence.

• Educate patients and families about available community resources and post-rehabilitation services to create effective support networks.

• Engage in discharge planning, ensuring seamless transitions to home or alternative care settings by effectively coordinating with other healthcare providers and the patient’s support network.
